Caveat Lens · The bundle

The summary report
and the school letter.

All four screeners, then one printable report that says what was checked, what we saw and which patterns are worth an evaluation, and a letter that asks the school, in writing, to evaluate.

The summary report

  • What was checked: each screener, the date, the grade and the time of year.
  • What we saw: every result with its band and the cutoff next to it.
  • Patterns worth an evaluation: combinations that commonly lead evaluators to look at dyslexia, dysgraphia, dyscalculia, language or processing speed, worded as patterns, never as a diagnosis.
  • What to do next: plain steps, including when to ask the school and when to screen again.
  • A clear statement of limits at the top, where a teacher or evaluator will see it.

It uses the first name you type, or "your child." Print it or save it as a PDF from the print window.

The letter to the school

  • A written request for a full and individual initial evaluation under the Individuals with Disabilities Education Act (IDEA).
  • It names the school's Child Find duty, asks for the consent form, notes the federal 60-day timeline from consent unless your state sets its own, and asks for a written explanation if the school declines.
  • Your concerns are drafted from the results; edit them freely and add what you see at home.
  • General federal terms only. It does not make claims about any state's rules and is not legal advice.

How to find a licensed evaluator

Who does evaluations, where to find names, how to check a license and what to ask before you book, plus free help from your state's Parent Training and Information Center.

What to bring

A checklist for the school meeting or the evaluator's first appointment: the report, your written request, work samples, what has been tried and family history.

Caveat Lens screeners are screening, not diagnosis. They do not diagnose dyslexia, dysgraphia, dyscalculia, a language disorder, ADHD or any other condition, and they do not replace an evaluation by a licensed psychologist, a neuropsychologist or a school evaluation team. There are no norms behind them; the bands are Caveat's own screening thresholds. If you are worried about your child, ask for an evaluation anyway.
